Sex Offender Registration Form
Sex Offender Change of Address Form May be printed for use when a registered sex offender needs to notify ACIC of an address change. Law enforcement may fax or mail the completed form to ACIC. The form must be signed by the offender in order for it to be processed promptly. Arkansas law requires the offender to inform ACIC of any change in residence not later than 10 days before establishing a new address. Anyone who is required by Arkansas law to register as a sex offender and fails to report any change of address shall be guilty of a Class C felony.
Sex Offender Acknowledgement Form (revised) Due to recent significant changes in state and federal law regarding the management of registered sex offenders, this revised form is provided for law enforcement use during the registration process.
