ASP-122 Individual Record Check

When an Arkansas background check is requested, include a properly completed ASP-122 request form; a check or money order in the amount of $25.00, made payable to the Arkansas State Police. DO NOT SEND CASH. A fingerprint card is NOT required to be submitted if only the Arkansas background check is requested. The results of the Arkansas background check will be sent to the person/entity to whom it was released.

ASP-122 Private Adoption Individual Record Check Request Form

Upon completion of the criminal record checks ran under A.C.A. 9-9-212 the Division of Arkansas State Police shall forward all information obtained to the court in which the adoption petition will be filed. We will not return the results to the applicant, only to the court.

To complete and FBI background check for this purpose please make sure you have a current transaction number provided by the Arkansas State Police for a live scan submission. Please see the Fingerprinting Information Services section for more information on submitting live scan fingerprints.
